**Job Overview:**
We are seeking a detail-oriented LVN, Intake Coordinator to join our healthcare team. The ideal candidate will be responsible for coordinating patient intake and care delivery, ensuring efficient and accurate service.
**Responsibilities:**
- Receive and enter patient information in electronic system.
- Verify insurance coverage and obtain necessary authorizations.
- Maintain confidentiality of patient information in compliance with HIPPA regulations
- Assist with medical records management including filing and organizing documents
- Answer phone calls and respond to inquiries from patients and other healthcare professionals when necessary.
- Provide administrative support.
- Collaborate with the Administrator to ensure smooth transitions.
- QA
- Audit charts for quality of care rendered; i.e. orders followed, appropriate utilization of visits, appropriate contacts with physician and other disciplines. Will be rated by supervisor based on findings.
- Assist in the field when necessary
- Other duties as assigned, will be dicusssed during interview
**Expreience**
- Prior experience in a healthcare setting, preferably with knowledge of Home Care Home Base and Epic software
- Familiarity with various medical procedures and terminology
- Strong communication skills to interact with patients and healthcare professionals
- Ability to multitask in a fast-paced environment
- Understanding of pain management protocols
- Qualifications:
- Must hold a current Licensed Vocational Nurse (LVN) license in the state of Texas
- Knowledge of EMR (Hospice MD)
- Strong knowledge of physiology and medical terminology
- Experience in hospice, geriatrics, senior care, and occupational health is preferred
EXPERIENCE AND REQUIRED SKILLS:
- 2 - 5 years' related experience; healthcare industry preferred.
- Experience with Medicare, Social Security and Medicaid systems a plus.
- Past patient interaction a plus.
- Excellent written and communication skills.
- A strong customer service philosophy.
- Strong organizational and time management skills.
- Ability to work independently.
- Proficient with PCs and Microsoft Office applications.
- Must be computer Savvy
- Valid Texas Driver's License
