Position Summary:

We are seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to join a multidisciplinary healthcare team in a hospital environment. This role involves providing comprehensive social work services to patients and families in an inpatient medicine and surgical setting. The ideal candidate will be highly organized, clinically skilled, and confident navigating complex care environments.

Key Responsibilities:

• Conduct psychosocial assessments and provide appropriate interventions for patients and families.

• Collaborate with physicians, nurses, and case managers to support care coordination and discharge planning.

• Assist patients and families in managing emotional, social, and environmental challenges related to illness and hospitalization.

• Provide crisis intervention, short-term counseling, and referrals to community resources as needed.

• Maintain accurate, timely, and thorough documentation in accordance with hospital policies and regulatory standards.

Qualifications:

• Minimum of 1 year of experience as a Licensed Master Social Worker.

• Prior experience in a hospital setting, particularly in inpatient medicine or surgery, is strongly preferred.

• Strong interpersonal, clinical assessment, and communication skills.

• Ability to manage multiple priorities and work effectively in a fast-paced environment.

Education & Licensure:

• Master’s degree in Social Work (MSW).

• Active Licensed Master Social Worker (LMSW) license issued by the New York State Education Department (NYSED).

• Primary Source Verification required.
