Overview
We are seeking a dedicated and organized Client & Care Manager to act as a vital bridge between clients, their families, and the care team. This role will ensure a smooth and effective delivery of care to all of our clients. The ideal candidate will focus on creating an engaging and efficient care experience for our clients as well as assessing which new or current staff members will compliment ongoing shifts with our clientele. This role is essential in fostering a positive company culture and enhancing both client and employee retention.

Our goal is to provide seniors with personalized services designed to support achieving or maintaining their desired level of independence. We spend time getting to know our clients and what is important to them. Together we craft a menu of services which are designed to give them support when and where it's needed!
Duties
- Create and maintain detailed schedules for clients, matching them with appropriate caregivers based on skillset, availability, and client preferences when needed.
- Utilize CRM systems for tracking, updating shifts, and scheduling client visits.
- Work hand in hand with Operations to coordinate clients and caregiver needs.
- Be primary contact between clients, caregivers, and the agency handling scheduling, inquiries, changes, and concerns.
- Data Entry and record keeping.
- Build and maintain strong relationships with clients, their families, communities, and the broader care team
- Evaluate clients physical, psychological, and social needs to develop personalized plan of care
- Visit clients monthly to assess if any changes to plan of care and or caregiving staff is needed
- Manage caregiver introductions to clients
- Handle client complaints and concerns
- Assist in managing referrals and new client onboarding
- Organize and manage various aspects of care, including medical appointments, transportation, medication management and communication with healthcare providers as needed
- Run errands and shopping for clients as needed
- Act as client’s advocate to ensure their needs are met and their rights are protected
- Participate in quality assurance activities to ensure high standards of care
- Adhere to all relevant state regulations and agency policies related to scheduling, client care, and handling of staff.
- Ensure compliance with all contractual obligations related to employees and clients
- Various administrative duties as needed/instructed such as answering phones, updating company calendar, compliance reviews, scanning, copying, etc.
- Must maintain privacy of clients at all times.
Experience
- Problem-solving and critical thinking skills
- Computer skills is an absolute must with experience working with CRM's.
- Needs to understand home care certification and state requirements.
- Experience working in home care office environment.
- Excellent organizational skills, attention to detail, and the ability to manage multiple priorities simultaneously.
- Strong communication skills, both verbal and written, to effectively engage with clients and existing and new caregiver staff.
