Essential Responsibilities and Duties:
- Initiate and maintain healthcare contacts within the assigned territory. Maintain contact with hospitals, physicians, physician’s staff, discharge planners, social workers.
- Serve as a liaison between patients, physicians and agency staff to ensure that the patient’s needs are being met.
- Pre-screen patients from physicians, hospitals, long-term care facilities and assisted living facilities for home health needs, eligibility and status, when referral is concurrent with discharge.
- Make marketing presentations to current and prospective clients to educate them on the agency’s services.
- Deliver orders to physician’s offices for signature and ensure timely return of the orders.
- Communicate with each referral source to ensure that the expectations of the referral sources and the needs of the patient are being met.
- Provide timely, accurate completion of reports, worksheets, and information as requested by the Administrator/designee.
- Participate in company-sponsored programs and meetings and maintain a networking base in the community through trade associations, business organizations and local association meetings.
- Maintain knowledge of the agency’s policies and procedures and Medicare regulations that are applicable to home health care.
- Able to work cooperatively as a member of a team.

Education and/or Experience:
- 2+ years of sales experience in home care and/or health-related field preferred
