Job Summary

The Admissions and Discharge Coordinator ensures a smooth transition for residents entering and exiting the facility. This role manages the intake process, facilitates discharges, and collaborates with families, referral agencies, and the treatment team to meet legal and organizational standards.

Essential Job Duties & Responsibilities

  • Oversee the 24/7 admissions process, including intake assessments, interviews, documentation, and orientation for new residents.
  • Facilitate discharge planning, including creating discharge summaries and coordinating aftercare resources.
  • Serve as the primary point of contact for referral sources and families during admissions and discharges.
  • Ensure compliance with regulatory requirements and documentation standards for all admissions and discharges.
  • Maintain accurate records of resident entry and exit dates, case notes, and necessary documentation.
  • Provide support to families and residents during transitions, addressing any questions or concerns.
  • Participate in treatment team meetings to coordinate admission and discharge planning.
  • Update internal systems and communicate changes in resident status to relevant staff.
  • Transport children to and from appointments as necessary

Other Job Duties & Responsibilities

  • Represent the facility in community outreach, networking, and partnership-building activities.
  • Participate in special projects or initiatives as assigned by senior leadership.
  • Act as a liaison between staff, residents, families, and other stakeholders.
  • Other duties as assigned

Job Qualifications

  • A bachelor’s degree in social work, counseling or related field
  • A minimum of two (2) years' experience in counseling, childcare, social work, and/or in providing therapeutic support services to children and families
  • An equivalent combination of education, training, and experience
  • Licensed Social Worker (preferred)

This job operates in a youth residential treatment facility. This role routinely uses standard office equipment. While performing the duties of this job, the employee is regularly required to speak and hear. The employee is frequently required to sit for extended periods of time, stand, walk, use hands and fingers, and reach with hands and arms. This position may encounter blood or other body fluids.
