Position Summary:
Toney Rest Home is seeking an accomplished and highly experienced Resident Care Coordinator with 15+ years of progressive experience in senior care. This pivotal role requires a compassionate leader who is not only dedicated to fostering growth within their team but also willing to actively participate in hands-on care when needed. The Resident Care Coordinator will be instrumental in ensuring the highest quality of resident care, leading and developing our care staff, and upholding Toney Rest Home's unwavering commitment to excellence.
Responsibilities:
- Care Leadership & Oversight:
- Oversee and manage all aspects of resident care, ensuring the delivery of personalized, compassionate, and high-quality services in accordance with established care plans, policies, and North Carolina regulations.
- Conduct regular assessments of residents' needs and adapt care plans as necessary, collaborating closely with residents, families, and healthcare providers.
- Monitor and evaluate the effectiveness of care interventions, making adjustments to optimize resident outcomes.
- Ensure accurate and thorough documentation of all resident care, medication administration, and significant events.
- Staff Training & Development:
- Develop, implement, and lead comprehensive training programs for all care staff, including new hire orientation and ongoing professional development, focusing on best practices in senior care, resident rights, and specialized care needs (e.g., dementia care).
- Provide continuous coaching, mentoring, and performance feedback to care team members, fostering a culture of continuous learning and improvement.
- Identify skill gaps and provide targeted training to enhance staff competency and confidence.
- Staff Supervision & Management:
- Directly supervise and schedule care staff (CNAs, Med-Techs, and other caregivers) across all shifts, ensuring appropriate staffing levels to meet resident needs.
- Conduct performance evaluations, address performance concerns, and implement disciplinary actions as needed, in collaboration with facility leadership.
- Foster a positive and collaborative work environment, promoting teamwork, respect, and open communication among all staff.
- Lead by example, demonstrating professionalism, empathy, and a strong work ethic.
- Medication Administration & Oversight:
- As a certified Med-Tech, expertly administer medications as prescribed by physicians, adhering strictly to all protocols, safety standards, and state regulations.
- Oversee and audit medication pass procedures performed by other Med-Techs, ensuring accuracy and compliance.
- Maintain meticulous medication records and inventory, managing reorders and disposals appropriately.
- Hands-on Support:
- Willingly provide direct resident care on the floor as needed, assisting with activities of daily living (ADLs), transfers, and other personal care tasks to support the team and ensure resident well-being, especially during peak times or staff shortages.
- Compliance & Quality Assurance:
- Ensure strict adherence to all federal, state, and local regulations pertaining to assisted living and memory care facilities in North Carolina, including those set forth by the NC Department of Health and Human Services (DHHS).
- Participate in and prepare for regulatory surveys and inspections, addressing any deficiencies promptly and effectively.
- Implement and monitor quality assurance programs to continuously improve resident care services and outcomes.
- Investigate and respond to resident and family concerns, incident reports, and complaints in a timely and effective manner.
- Collaboration & Communication:
- Serve as a primary point of contact for residents, families, and external healthcare providers regarding resident care needs.
- Collaborate effectively with the Executive Director, nursing staff, and other department heads to ensure seamless and coordinated care.
- Participate in resident care conferences and interdisciplinary team meetings.
Qualifications:
- [Required]: Current and active Certified Nursing Assistant (CNA) certification in North Carolina.
- [Required]: Current and active Medication Technician (Med-Tech) or Certified Medication Aide (CMA)certification in North Carolina, specifically for Adult Care Homes (Assisted Living).
- [Required]: Minimum of 15+ years of progressive experience in direct resident care, with at least 5 years in a supervisory or leadership role within an assisted living, long-term care, or residential care setting.
- Demonstrated commitment to professional growth and continuous learning in senior care.
- Proven ability to train, supervise, and motivate a diverse team of care professionals.
- Exceptional understanding of best practices in elder care, including dementia care.
- Thorough knowledge of North Carolina state regulations governing assisted living facilities and medication administration.
- Strong leadership, organizational, and problem-solving skills.
- Excellent verbal and written communication skills, with the ability to interact empathetically and effectively with residents, families, staff, and external stakeholders.
- Ability to remain calm and effective in stressful situations.
- Proficiency in electronic health records (EHR) systems and basic computer applications.
- Physical ability to perform the duties of the position, including lifting, bending, and assisting residents as needed.
- Must successfully pass a comprehensive background check and drug screening.
