Role Description

This is a contract, on-site role for an Inpatient Social Worker located in New York, NY. The Inpatient Social Worker will be responsible for providing support to patients and their families, including conducting assessments, developing care plans, and coordinating resources. This role involves close collaboration with healthcare teams to address the social, emotional, and economic needs of patients. The Social Worker will also advocate for patient needs and work to ensure they receive comprehensive, integrative care.

Qualifications

• Experience in conducting assessments and developing care plans
• Ability to coordinate resources and collaborate with healthcare teams
• Strong advocacy skills and ability to address social, emotional, and economic needs of patients
• Excellent interpersonal and communication skills
• Master's degree in Social Work (MSW) or related field
• Experience in a healthcare setting is a plus
