Position Summary:
The Social Worker is responsible for meeting the psychosocial needs of residents in a long-term care setting. This role ensures compliance with all applicable federal and state regulations, supports residents and families during transitions, and promotes overall quality of life through counseling, care planning, and advocacy.
Key Responsibilities:
Complete and maintain comprehensive psychosocial assessments for all residents upon admission, quarterly, annually, and with significant changes
Develop and implement individualized care plans that address psychosocial needs, and update them as necessary
Provide emotional support, counseling, and crisis intervention for residents and families
Participate in interdisciplinary care plan meetings and document social service input
Assist residents and families with discharge planning and referrals to community resources
Respond to and resolve resident grievances and concerns in accordance with resident rights
Maintain timely, accurate, and regulatory-compliant documentation in resident medical records
Ensure compliance with federal and state regulations, including CMS and OBRA guidelines
Promote and uphold the rights and dignity of each resident
Requirements:
Bachelor’s Degree in Social Work (BSW) from an accredited institution
Active New Jersey certification or license to practice as a Social Worker (e.g., CSW, LSW)
Minimum of 1 year of experience in a skilled nursing facility or long-term care setting
Strong working knowledge of long-term care regulations, including MDS, care planning, and documentation standards
Excellent interpersonal, communication, and conflict-resolution skills
Ability to manage time, prioritize tasks, and work collaboratively in a multidisciplinary team
