Job Purpose:
Key Responsibilities:
- The Care Manager at Behavioral Health Services Inc is responsible for coordinating and managing patient care plans to ensure the effective delivery of healthcare services. This role involves collaborating with healthcare providers, patients, and their families to facilitate comprehensive care and optimize patient outcomes.
Key Responsibilities:
- Develop and implement individualized care plans for patients, ensuring that all healthcare needs are addressed.
- Coordinate with healthcare providers, including doctors, nurses, and therapists, to ensure seamless delivery of services.
- Monitor patient progress and make necessary adjustments to care plans to improve health outcomes.
- Educate patients and their families about treatment options, care plans, and available resources.
- Facilitate communication between patients, families, and healthcare providers to ensure understanding and cooperation.
- Maintain accurate and up-to-date patient records and documentation.
- Advocate for patient needs and preferences in the healthcare system.
- Ensure compliance with healthcare regulations and standards.
- Participate in case conferences and team meetings to discuss patient care and progress.
- Provide support and guidance to patients and families navigating the healthcare system.
Required Education:

Required Skills and Abilities:
- Bachelor's degree in Nursing, Social Work, Psychology, or a related field.
Required Experience:
- Minimum of 2 years of experience in case management or care coordination within a behavioral health setting.
- Experience working with diverse populations and understanding their unique needs.
- Proven track record of developing and implementing care plans for individuals with mental health challenges.
Required Skills and Abilities:
- Strong understanding of behavioral health concepts and practices.
- Excellent communication and interpersonal skills to effectively interact with clients, families, and healthcare professionals.
- Ability to assess client needs and develop personalized care plans.
- Proficient in using electronic health records and other relevant software.
- Strong organizational skills and attention to detail.
- Ability to work independently and as part of a multidisciplinary team.
- Knowledge of local community resources and ability to connect clients with appropriate services.
- Ability to handle sensitive information with confidentiality and professionalism.
