Job Description

The Addiction Response Team (ART) is a multidisciplinary service that provides acute substance use disorder (SUD) care to patients in Emergency Departments (EDs), inpatient (IP) units, and SUD Walk-in clinics, throughout the hospital. The team is led by the ART Associate Medical Director and includes medical providers with addiction training, social workers, peer counselors, and community health workers. Together they provide expertise in medications for AUD and OUD, SUD screening, brief intervention, referral to ongoing treatment (SBIRT), peer support, care coordination, and harm reduction.

Under general supervision, with moderate latitude for independent judgment and decision making, in addition to performing the duties of the lower Assignment Levels at a higher level either supervises the duties of social work staff or performs tasks in a specialized clinical and/or programmatic area.

Summary Of Essential Duties And Responsibilities

  • May provide direct clinical supervision and guidance to social work staff, screen and assign cases, schedule staff, read and review staff work, evaluate work performance and/or maintain time and leave records.
  • As a Licensed Clinical Social Worker (LCSW), provides and documents clinical supervision of NYC H+H Licensed Master Social Workers (LMSWs) toward advanced Social Work licensure in alignment with the all relevant professional standards and practices, and in accordance with all applicable NYS licensure laws.
  • Maintains individualized supervision logs documenting clinical supervision and ensures timely completion of attestation forms required by NYSED Office of the Professions to support LCSW licensure applications, as applicable.
  • May conduct or lead special studies and projects including the collection and evaluation of data related to vulnerable patient populations and prepare and present reports related to these service populations.
  • Establishes and maintains professional social work standards of performance concerning interview techniques and methods and practices used in preparing clinical assessments, developing treatment plans, and in documenting case histories
  • Interprets clinical information based on case data, histories, statistics, etc.; prepares and submits comprehensive reports of case findings including chronologies of medical data and social factors necessary to support recommended courses of action.
  • Ensures compliance with accepted social work practices and principles and operational adherence to government rules and regulations, including compliance with reporting requirements.
  • Participates in conferences, committees and meetings on- and off-site with supervisory/managerial staff, other clinical professionals and with representatives of government and other agencies and/or community organizations.
  • Analyzes population health issues relevant to the area of responsibility and serves as a liaison to community agencies and/or groups.
  • In the temporary absence of the supervisor, may perform the duties of that position.
  • Under the clinical direction of the ART Associate Medical Director/Associate Director of Mental Health, serve as the facility's SUD ambassador providing substance use and harm reduction services throughout the hospital, prioritizing SUD acute/sub-acute care patients
  • Provides timely and complete screening, brief intervention, and/or psychosocial assessment to patients at risk for a SUD located within an acute care setting: ED, IP, Bridge, or Ancillary/Withdrawal outpatient (OP) clinic.
  • Assists with the service needs of all medication assisted treatment (MAT) patients, initiating priority outreach and follow up services for patients with Opioid Use Disorder (OUD)
  • Assesses and identifies the patient’s level of care, providing recommendations or referrals to a higher level of care as needed (inpatient detox or rehab).
  • Completes intakes for the facility’s H+H SUD outpatient clinic as necessary, coordinating follow up services.
  • Conducts patient treatment/discharge/aftercare planning in collaboration with primary care team as it relates to SUD.
  • Provides direct care individual, group counseling, and/or outreach services; making referrals to community-based providers as appropriate.
  • Escorts patients for SUD placement interviews/discharge as appropriate and/or indicated by supervisor/designee.
  • Provides opioid overdose education and harm reduction resources including naloxone kit and fentanyl/xylazine test strips to patients and family members.
  • Interviews patients’ relatives, facility staff, and members of the community regarding patients’ ability to function in the community, as appropriate.
  • Participates in interdisciplinary Performance Improvement (PI) and/or Quality Improvement (QI) projects regarding social work practice on complex issues.
  • Attends ART case conferences, trainings, and meetings such as within acute care/sub-acute service areas as well as rounds as it relates to SUD; participates in work of selected staff committees
  • Orients/trains and educates staff members of other professional disciplines on Social Work concepts and functions. Serves as liaison with community agencies; researches, develops and facilitates the utilization of community resources.
  • Reports suspected cases of child abuse or neglect to the New York Central Registry.
  • Works closely with other substance abuse treatment facilities (detox, rehab centers, residential programs) to facilitate the next level of care for SUD patients.
  • Ensures compliance with accepted social work practices and principles and operational adherence to government rules and regulations.
  • Keeps electronic records, maintains referral lists, prepares reports, and related paperwork, ensuring patient confidentiality
  • Reports suspected cases of child abuse or neglect to the New York Central Registry
  • May maintain a small caseload of SUD “Bridge Clinic” patients providing counseling services until long-term SUD treatment services are established
  • May provide coverage in any of the facility’s other SUD service areas as needed
  • Other duties as assigned by supervisor/designee

Minimum Qualifications

For Assignment to Level II

  • Valid license as an LMSW issued by the NYSED; or
  • Valid license as an LCSW issued by the NYSED.
