Job Summary:

Position Overview:
We are seeking a dedicated Social Worker (MSW) to work closely with the Director of Patient Care Services and a Physician to provide compassionate and comprehensive care to our patients. The ideal candidate will assist in coordinating care plans, providing emotional and social support, and ensuring that patients and their families have access to the necessary resources and services to improve overall well-being.

Key Responsibilities:

  • Collaborate with the Director of Patient Care Services and the attending Physician to assess patient needs and develop individualized care plans.
  • Provide counseling, emotional support, and guidance to patients and families dealing with chronic illness, mental health challenges, or other social concerns.
  • Assist in discharge planning and coordinate with community services, healthcare providers, and other agencies to ensure continuity of care.
  • Conduct psychosocial assessments to evaluate patient circumstances and needs.
  • Act as a liaison between the patient, family, healthcare providers, and community resources to ensure a comprehensive approach to patient care.
  • Maintain up-to-date documentation on patient interactions, care plans, and follow-up activities, ensuring compliance with legal and ethical standards.
  • Participate in interdisciplinary team meetings to review patient care plans and progress.
  • Stay current on social work best practices and developments in healthcare policies that impact patient care.

Qualifications:

  • Master of Social Work (MSW) from an accredited institution.
  • Licensed Social Worker (LMSW or LCSW) in the state of [Insert State].
  • 2+ years of experience in a healthcare setting (preferred).
  • Experience working in a multidisciplinary healthcare team environment.
  • Knowledge of community health resources, case management, and discharge planning.
