Job Details

If you are a highly skilled Registered Nurse who excels at influencing care decisions, meeting deadlines, and collaborating with an interdisciplinary care team, ChristianaCare HomeHealth is the place for you. The primary duties of this role include providing clinical mentorship to ensure plan of care accuracy, utilization management, and timely submission of documentation to support success under the new Medicare payment structure - Patient Driven Groupings Model (PDGM).


ChristianaCare HomeHealth is seeking a Full-Time RN to work in either our Smyrna or Avenue North office, providing mentorship to clinicians to ensure timely and accurate plan of care development and maintenance. The successful candidates will work Monday through Friday, 9 am to 5:30 pm, with a weekend rotation approximately once every 7 weeks. After orientation, there may be an opportunity to work weekends remotely. No holiday requirements.

Duties and Responsibilities:

  • Ensuring visit plans and ongoing care are appropriate using principles of Case Management/Utilization Management
  • Collaborating with Quality, Education, and Clinical Operations on performance improvement
  • Providing expert guidance on accurate completion of OASIS assessments
  • Working with medical coding staff to ensure proper documentation in medical records

Requirements:

  • Delaware RN license
  • BSN degree
  • OASIS certification (must be obtained within one year if not already certified)
  • Case Management or Ambulatory Care Certification preferred
  • Two years of Home Health experience preferred; hospice or insurance case management considered
