Job Title: Patient Care Coordinator – Medicare Wellness (Full-Time)

Location: MacGregor Medical Center
Schedule: Monday through Friday (Full-Time)
Pay: $16.00 per hour

MacGregor Medical Center is a busy and respected physician-owned medical office serving both commercial and Medicare patients. We are dedicated to providing high-quality, patient-centered care with a strong focus on prevention and wellness. Our Medicare Wellness Center supports our senior patient population by helping them stay on top of their preventative health through Annual Wellness Visits and other Medicare-based services.

Position Overview:
We are currently hiring a Patient Care Coordinator to join our team full-time in our Medicare Wellness Center. This role is dedicated to supporting our Medicare patients by scheduling Annual Wellness Visits (AWVs), tracking important health metrics, offering resources, and helping resolve issues or concerns that may arise.

Key Responsibilities:

• Schedule and coordinate Medicare Annual Wellness Visits (AWVs)
• Assist patients with understanding and accessing Medicare-specific resources
• Track and manage patient wellness metrics and care gap closures
• Work closely with providers and clinical staff to ensure continuity of care
• Follow up with patients regarding appointments, screenings, and wellness reminders
• Answer questions and resolve concerns related to Medicare services
• Maintain accurate records in the scheduling and electronic medical records systems

Qualifications:

• High school diploma or equivalent (required)
• Experience in a medical office or healthcare setting preferred
• Familiarity with Medicare and preventative care services is a plus
• Excellent communication, organization, and customer service skills
• Comfortable using computers and electronic medical record systems
• Compassionate, patient-focused attitude with a team-oriented mindset
