Job Summary
The Home Health Physical Therapy Manager plays a critical leadership role in overseeing the delivery of physical therapy services within a Medicare-certified home healthcare agency. This position ensures therapy services meet clinical quality standards, comply with all CMS regulations, and support patient recovery and independence at home. The manager leads a team of physical therapists and therapy assistants, ensuring optimal coordination of care and consistent regulatory compliance.
Key Responsibilities
Clinical Oversight and Compliance
Direct the delivery of physical therapy services in accordance with Medicare Conditions of Participation (CoPs), CMS guidelines, and agency policies.
Review and approve patient evaluations, plans of care (POCs), and re-certifications for clinical accuracy, timeliness, and documentation standards.
Ensure therapy documentation supports medical necessity and meets audit-readiness criteria for Medicare and third-party payers.
Serve as a clinical resource for therapy staff on CMS updates, OASIS requirements, and patient-centered outcomes.
Team Leadership and Supervision
Supervise and mentor physical therapists, PTAs, and contracted therapy personnel.
Coordinate caseload assignments to balance clinician workload and ensure geographic efficiency.
Provide onboarding, skills assessment, performance evaluation, and ongoing education for therapy staff.
Interdisciplinary Collaboration
Participate in care coordination conferences and interdisciplinary case discussions.
Collaborate with nursing, occupational therapy, speech therapy, and medical social work to deliver integrated care.
Ensure continuity of care during start-of-care (SOC), recertifications, transfers, and discharge planning.
Quality Assurance and Program Development
Participate in Quality Assurance and Performance Improvement (QAPI) programs by analyzing clinical outcomes, adverse events, and documentation trends.
Identify opportunities for clinical improvement and staff development through audits and data review.
Assist in developing policies and workflows to enhance care quality and regulatory readiness.
Administrative and Operational Duties
Contribute to department budgeting, productivity tracking, and Medicare utilization review.
Ensure timely submission of documentation to support billing cycles and reduce payment delays or denials.
Monitor and ensure therapists meet deadlines for OASIS assessments, care plan updates, and re-evaluations.
Qualifications
Education and Licensure
Graduate of an accredited physical therapy program (Bachelor’s, Master’s, or DPT).
Licensed Physical Therapist in the State of [Insert State].
CPR/BLS certification required.
Experience
Minimum 3–5 years of clinical experience in home health physical therapy, including Medicare patients.
Minimum 2 years in a supervisory or leadership role preferred.
Strong knowledge of Medicare billing rules, OASIS-E, Plan of Care requirements, and CMS audit preparation.
Skills
Proven leadership, mentoring, and staff development abilities.
Proficiency in EMR systems (e.g., Homecare Homebase, Kinnser, WellSky).
Understanding of PDGM (Patient-Driven Groupings Model) and therapy contribution to reimbursement.
Excellent communication, critical thinking, and clinical documentation skills.
Working Conditions
