Pay Options:
$2,150/week = $30/hr on W2 + $1,100/week (non-taxable per diem)
$1,960/week = $56/hr on W2 (taxable, no per diem)

Position Summary:

The Certified Social Worker provides therapeutic intervention and comprehensive social work services to patients and their families within the hospital. This role ensures seamless inpatient care and continuity post-discharge by coordinating psychosocial support and discharge planning services.

Essential Qualifications:
Master’s Degree in Social Work (MSW) – Required
Active NJ Social Work License (LSW or LCSW) – Required
Recent hospital-based experience required
Must have inpatient experience with a focus on discharge planning

Key Responsibilities:
Assess psychosocial needs and develop care plans aligned with patient medical treatment
Interview patients, families, or significant others to identify needs linked to social determinants of health
Provide input during multidisciplinary team rounds to ensure coordinated care
Act as an advocate and resource for advanced care planning and psychosocial support
Support patients through illness, hospitalization, and life transitions
Coordinate psychosocial discharge plans including referrals to rehab, skilled nursing, hospice, or housing solutions
Document interventions and assessments per hospital policy and regulatory standards
Develop community-based social service networks and assist patients in accessing benefits and transitional services
Participate in continuous improvement of social work services and outcomes
Maintain confidentiality and professional ethics at all times

Skills & Competencies:
Strong communication, negotiation, and conflict resolution abilities
Familiarity with federal and state regulations related to hospital social work
Knowledge of current trends in healthcare and social work technologies
Ability to handle rapid change and contribute to clinical improvement processes
Physical Requirements:

This position requires full body mobility, including standing, walking, bending, lifting (up to 50 lbs), and manual dexterity. Visual and auditory acuity required. Reasonable accommodations may be provided.
