Job Description:
The Care Case Manager is an integral part of Clark's wrap-around support services for students, and will support the Director of Care Case Management with the facilitation of non-clinical interventions and upstream solutions that foster student well-being, retention, and persistence. The Case Manager will provide coordination of services and support to students experiencing academic, personal, and/or medical difficulties to assist them in removing barriers to their success and carry a revolving case load for students. In the absence of the Director, the Case Manager will facilitate the Care Leads meetings and ensure coordination amongst the various staff and departments that provide support and follow-up for students. The Case Manager will serve as a primary point of contact for students looking to take a leave of absence from the institution and will coordinate communication and support to students returning from time away from Clark to ensure a smooth and successful re-entry into the community.

Job Description:

Case Management and Student Support [40%]
- Coordinate follow-up and basic case management support for students of concern through one on one in-person or virtual meetings utilizing the NABITA Risk Rubric, particularly as it pertains to acute mental health problems and academic related issues. Meet regularly with students.
- Coordinate with faculty, staff, families, and external agencies to assess student needs and develop holistic Care plans through electronic communication and in person/ virtual meetings.
- Collaborate, consult, train and evaluate non-clinical care managers, ensuring they are equipped to manage complexity of cases effectively and professionally.
- Maintain accurate, timely, and confidential records of student cases using Advocate software, adhering to compliance with FERPA and related regulations.
- Train and supervise graduate student interns on triaging case referrals, privacy requirements, risk rubric utilization and care case monitoring to track outreach and interventions for positive student outcomes.
- Act as a liaison to academic departments, Health Services, Counseling & Personal Growth, University Police, Title IX, Athletics, Dean of the College, and other internal and external partners regarding case management and student support.
- Serve as a key administrator of the Advocate CRM as it relates to care case management. Conduct semesterly audit of systems processes and assist with maintenance as needed.
- Assist the Director with developing and updating systems reports to track and monitor pending cases and trends in student concerns. Assist in the timely communication of this information to Case Leads.
- Communicate regularly regarding students of concern, including elevating students to BIT
- Participate in department review of policy regarding case management and provide recommendations in alignment with best practices.
Student Leave of Absence and Re-entry Oversight [25%]
- Act as primary point of contact for students seeking a leave of absence or medical leave of absence from the university.
- Facilitate Return from Leave of Absence communication campaign hosted through Insight CRM.
- Act as a liaison to Center for Counseling and Personal Growth, Health Services and Dean of Student Success office regarding students returning from a Medical Leave of Absence, ensuring documentation is received from every student and communicated to the appropriate office to conduct a re-entry meeting.
- Conduct re-entry meetings with students who return from voluntary leave of absence to confirm they are adjusted back to campus and provide additional support and resources as needed.
Program Development [25%]
- Work with Health and Well-Being focused teams (Counseling and Personal Growth, Wellness Education, Health Services), the Office of Academic Engagement and Transition, and other academic support focused spaces to build out co-curricular programming to develop upstream intervention and solutions to minimize stress on down-stream supports and resources
- Prepare and deliver presentations to students, faculty, staff, and families on the Care network and training related to supporting and referring students of concern.
- Participate in department review of policy regarding case management and provide recommendations in alignment with best practices.
Participate in the growth and development of the Division and Clark University [10%]
- Support the on-boarding and off-boarding process for students, including having an active presence at signature events.
- Support critical retention and student success initiatives.
- Serve as a key leader in university health and well-being initiatives, programs, and services
- Support key initiatives and experiences within Student Success and the University, such as opening weekend, orientation, commencement, family weekend, traditional events, etc.
- Promote efficiency and courtesy throughout the University community, ensuring consistency with Student Success' mission and values.
- Remain cognizant of the University's mission and identity, the constant promotion of diversity, equity, and inclusion, and the importance of student voices.
- Communicate effectively with supervisor concerning pertinent departmental and University matters.
- Other duties as assigned that are in the best interest of the division.
Requirements:
Required Qualifications
- Master's degree in counseling, social work, student affairs, higher education, or a related field.
- Minimum 1-2 years of experience working with students in a higher education setting, including case management and crisis response.
- Demonstrated ability to manage and prioritize multiple cases simultaneously.
- Demonstrated knowledge of student development theory, Care coordination
- Ability to lead with a trauma-informed, culturally competent, and equity-minded approach with a commitment to diversity, equity and inclusion
- Strong critical thinking, analytical and problem-solving skills.
- Strong interpersonal, organizational, and both written and verbal communication skills.
- Demonstrated experience and commitment to supporting diverse and underrepresented populations and creating inclusive, safe and welcoming spaces.
- Experienced in conflict resolution/de-escalation and the ability to respond to crisis calmly and effectively
- Ability to work independently and take initiative, as well as collaborate with others.
Preferred Qualifications
- Familiarity with NABITA standards and the practices of Behavioral Intervention Teams.
- Experience with Advocate case management software and/or Banner.
- Certification in case management, wellness coaching or related training on motivational interviewing, appreciative inquiry and transtheoretical models of change and motivation.
- Experience in a primarily residential liberal arts setting.
