We're seeking a Part-Time Dietitian to join our dynamic team at our premier Skilled Nursing Facility, Martin Health Center, where excellence in care is our standard.
The Dietitian will be required to evaluate and screen nutritional status of individuals in accordance with established protocol. The Dietitian will be responsible for planning and implementing nutrition therapy for patients plus individual and group nutrition education and counseling. The Dietitian will be responsible for monitoring nutrition programs, facilities, and processes. The Dietitian is expected to participate as part of a health care team.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
· Evaluates and assesses nutrition status of residents, and screens residents for nutritional risk in accordance with established professional protocols and standards, including licensure standards for Martin Health Center (SNF).
· Participates in MDS process as required in Martin Health Center (SNF).
· Plans and initiates medical nutrition therapy to residents if indicated by resident requirements and clinical protocol.
· Educates and counsels residents on nutritional issues, either individually or in groups.
· Contributes to resident care plans for the licensed areas.
· Communicates individual nutritional care plans to other health care providers as a member of a health care team.
· Completes thorough documentation, including charting patient progress, dietary assessments, and evidence-based nutrition recommendations, in accordance with regulatory and organizational standards.
· Maintains professional growth and development through completion of required departmental competencies.
· Performs miscellaneous job-related duties as assigned.
This part-time position will be 12 hours per month with flexibility. This is an hourly position with a starting pay range of $28-$31 per hour.
MINIMUM QUALIFICATIONS:
- Credentialed Registered Dietitian (RD) through the Commission on Dietetic Registration. Holds Dietitian Licensure for the State of IL.
- Bachelor's degree(s) from an accredited institution required.
- Clinical experience preferred.
KNOWLEDGE, SKILLS AND ABILITIES:
· Ability to comply with HIPPA guidelines and regulations.
· Ability to develop and deliver presentations.
· Ability to effectively communicate medical information, test results, diagnoses and/or proposed treatment in a manner easily understood by the client.
· Knowledge of clinical nutrition theory and practice.
· Ability to draw conclusions and make recommendations based on research data and findings.
· Ability to resolve interpersonal problems, disputes, and issues.
