Job Summary:

Aspen Meadows Health and Rehabilitation Center is currently seeking a Social Worker to assist with maintaining the social service program designed to meet the individual physical, mental, and psychosocial needs of residents.

Essential Functions:

1. Work with residents, families, and community support to complete social service history, discharge plans, and develop interdisciplinary care plans to meet the bio-psychosocial needs of residents.


2. Assist residents and responsible parties in locating and utilizing financial, legal, mental health, and other community resources.


3. Coordinate and/or provide health and mental health social work services to help residents attain or maintain the highest practical mental and psychosocial well-being, and ensure residents with mental or psychosocial difficulties receive appropriate treatment and services.


4. Provide resource support to staff involved in behavioral interventions for residents.


5. Coordinate safe discharge planning and follow-up services in collaboration with the interdisciplinary team.

Knowledge, Skills & Education:

1. Required: Licensed in Montana as a Social Worker.


2. Effective communication skills with residents, families, and staff.


3. Preferred: One year of relevant experience in long-term care or a related field.
