Job Title: HCV Nurse Care Manager (NCM)
Reports To: Project Director
Location: Alliance Care 360 – Multi-site (includes mobile, clinic, and telehealth settings)
FLSA Status: Full-Time, Exempt (100% Level of Effort)

Position Overview:

The HCV Nurse Care Manager (NCM) is responsible for coordinating the integrated medical, behavioral, and psychosocial care needs of individuals diagnosed with Hepatitis C (HCV), with or without co-occurring HIV, substance use disorder (SUD), and serious mental illness (SMI). This role ensures a low-barrier, person-centered approach to treatment and actively supports same-day diagnosis, linkage to care, medication access, and adherence using evidence-based models.

Key Responsibilities:

  • Care Coordination:

Develops and monitors comprehensive care plans that integrate HCV, SUD, and behavioral health services, working closely with peer navigators, physicians, and behavioral health teams.

  • Treatment Adherence Support:

Oversees the implementation of medication adherence tools including the Medisafe app, peer-assisted Directly Observed Therapy (DOT), and MedFriend alerts. Conducts check-ins to troubleshoot access or adherence barriers.

  • Client-Centered Navigation:

Assists clients in navigating pharmacy systems, Medicaid/Medicare coverage, Ryan White benefits, and other wraparound services to initiate and complete Direct-Acting Antiviral (DAA) treatment.

  • Outreach & Engagement:

Participates in mobile clinics, shelter-based programs, and community events to screen, educate, and enroll high-risk individuals. Provides education on HCV, prevention, and cure.

  • Clinical Documentation & Reporting:

Maintains accurate, timely documentation in the electronic health record (EHR), tracks clinical outcomes, and contributes to grant-required reporting metrics, including adherence and SVR (sustained virologic response) rates.

  • Collaboration:

Works in multidisciplinary teams with Ryan White providers, Project ECHO clinical hubs, housing and mental health partners, and state/county public health systems.

Minimum Qualifications:

  • Registered Nurse (RN) or Advanced Practice Registered Nurse (APRN) licensed in Illinois
  • At least 3 years of experience in infectious disease, addiction medicine, or community health
  • Knowledge of Hepatitis C treatment protocols, medication access programs, and harm reduction
  • Strong communication and cultural competency skills; able to work with vulnerable populations
  • Experience with Medicaid/Medicare billing and EMR platforms preferred
