Job Summary
Housing Case Manager is responsible for providing case management, supportive services, and housing support for young adults ages 18-24. The Case Manager helps prepare program participants for independent living by ensuring they are making progress toward life skills, education, employment, and permanent housing goals
Essential Duties and Responsibilities
- Provide weekly face-to-face contact with each participant, including home visits in scattered-site or remote settings.
- Develop, implement, and monitor Individual Case Plans in coordination with the participant and referring county agencies.
- Support youth in developing independent living skills such as budgeting, cooking, cleaning, self-care, and time management.
- Collaborate with county social services, probation officers, schools, and community partners.
- Maintain accurate and timely documentation and case notes in accordance with agency policy and/or licensing requirements.
- Conduct regular Life Skills assessments and facilitate workshops or connect youth to appropriate training opportunities.
- Assist youth with education planning including high school completion, vocational training, and college access.
- Support employment readiness by assisting with job applications, interview prep, and job retention skills.
- Provide crisis intervention and conflict resolution support when needed.
- Monitor youth adherence to program rules, address behavioral concerns, and work to maintain housing stability.
- Attend multidisciplinary team meetings.
- Maintain participant files, including copies of leases, TILPs, court reports, and other required forms.
Job Types: Full-time, Part-time
Pay: $22.00 - $24.00 per hour
Expected hours: 40 per week
