Position Summary:
Joining our team at Plaza del Sol Family Health Center in Corona, Queens, the Nurse Care Manager (Registered Nurse) works to support primary care patients that were referred for Obstetrics (OB) Services and is a liaison to facilitate patient/provider communication. The Registered Nurse performs required assessments and provides direct patient care services to targeted patients to ensure they adhere to the treatment plan, including patient education.
The Nurse Care Manager develops, evaluates, and implements individualized Care Plans in accordance with the health care needs as directed by the PCP. Educates the patients and their families on how to follow their care plans and updates them as needed. Provides linkages for services including but not limited to Nutrition Services, Mental Health Services, or Case Management.
Key Duties:
  • Coordinates the patients care under the direction of the PCP with an emphasis on OB Services
  • Develops a Care Plan with patients and family based on assessment and updates Care Plan to reflect patient treatment goal, status achievement, barriers and intervention.
  • Operates doppler.
  • Completes required documentation for each action taken on behalf of the patients.
  • Provides education to patients related to health literacy, including aspects of pregnancy, childbirth, postpartum care, family planning, fertility and health related decisions.
  • Establishes self-management goals with patients, tracks goals, and provides tools and resources according to patients' needs.
  • Communicates with patients' providers both internally and externally to UHP, including the Health Plan to coordinate patients' care.
  • Provides post partum follow up for mother and newborn.
  • Attends training as assigned to assure up to date knowledge in care coordination/care management.
Minimum Qualifications:
  • Bachelors in Nursing (BSN) required with 2-5 years of established work experience as a Registered Nurse.
  • Bilingual English/Spanish required.
  • Exemplary communication skills.
  • Fundamental understanding of primary care/ambulatory nursing. Background in OB/GYN or L&D is a huge plus.
  • Passion for community health, passion for women's health, passion for helping and advocating for other.
Starting Compensation: Minimum $97,000/annum; Maximum $102,000/annum.
Work Environment Type, Location, and Schedule:
Onsite, Corona, Queens, NY, Monday to Friday 8:00am-5:00pm. (Occasional Saturdays with a day off during week)
