Job Summary
The Care Manager plays a vital role in coordinating patient care and ensuring that individuals receive comprehensive and effective healthcare services. This position involves working closely with patients, families, and healthcare providers to develop and implement personalized care plans. The Care Manager will focus on optimizing patient outcomes through effective utilization management, education, and support across various healthcare settings.
Duties
- Assess patient needs and develop individualized care plans that address physical, emotional, and social aspects of health.
- Coordinate with multidisciplinary teams to ensure seamless transitions of care across different settings, including Level II trauma centers and primary care facilities.
- Monitor patient progress and adjust care plans as necessary to meet evolving health needs.
- Provide education and support to patients and their families regarding treatment options, resources, and self-management strategies.
- Advocate for patients within the healthcare system to ensure they receive appropriate services and support.
- Collaborate with behavioral health specialists to address mental health concerns as part of comprehensive patient care.
- Document all interactions, assessments, and care plan updates accurately in the electronic health record system.
Skills
- Strong knowledge of patient care practices, including experience in geriatrics, PICU, or trauma settings.
- Proficient in utilization management principles to optimize resource allocation while maintaining high-quality patient service.
- Understanding of physiology to effectively assess patient conditions and needs.
- Excellent communication skills for interacting with patients, families, and healthcare professionals.
- Ability to work collaboratively within a multidisciplinary team environment.
- Strong organizational skills with attention to detail in managing multiple patient cases simultaneously.
- Compassionate approach towards patient interactions with a focus on empathy and support throughout the care process.
