  • Location: Costa Mesa, CA, this is not a remote or hybrid position.
  • Schedule: 8 a.m. to 4:30 p.m. Monday through Friday.

The Clinical Care Coordinator works closely with the clinical staff to ensure patients are provided with quality care and service, providers, and patients. The Clinical Care Coordinator will schedule appointments, verify insurance benefits and be responsible for administrative duties of the project.

Duties will include:

  • Demonstrate professionalism while embodying the mission, purpose, values, and beliefs in everyday language and contact with the internal and external stakeholders.
  • Familiarity and comfort working with issues related to substance abuse, mental health diagnosis, and history of trauma.
  • High level of motivation and self-starting demeanor with the ability to coordinate multiple projects and task simultaneously in a high-pressure environment.
  • Provide exceptional customer services to all patients and family support systems.
  • Work collaboratively and effectively within the team and contribute to building a positive culture.
  • Maintain awareness of the admission processes.
  • Manage facility communication.
  • Monitor and respond to patient email portal.
  • Assist patients with preliminary medical history, vital sign measurement, and data entry into electronic health records.
  • Document patient information accurately and efficiently.
  • Schedule patient appointments, manage phone calls, and coordinate patient flow.
  • Prioritize and manage urgent appointments requests.
  • Handle patient registration, insurance verification, and medical record management.
  • Collaborate with facility staff to ensure seamless patient appointments.
  • Schedule mobile nurse teams to facility locations.
  • Collaborate with the business development team to facilitate new referrals and partnerships.
  • Assist with discharge processes and follow up with calls to ensure ongoing care needs are met.
  • Maintain confidentiality according to federal guidelines.
  • Attend required department meetings and training.

What you will need:

  • High school diploma or equivalent.
  • Experience working in a healthcare setting.
  • Familiarity with electronic health records (EHR) systems.
  • Computer literacy and working knowledge of desktop software.
  • Experience using a cell phone, laptop, and various technology.
  • Ability to prioritize and handle multiple tasks efficiently.
