Position Summary

The ECM Lead Care Manager (LCM) under the direct supervision of the ECM Director is primarily responsible for coordinating and implementing organization-wide Enhanced Care Management (ECM). The ECM Lead Care Manager works closely with members’ health professionals (medical, dental, behavioral health, and vision teams), to develop and implement personalized care management plans and support with tracking progress and adjusting as necessary. The ECM Lead Care Manager conducts initial and periodic assessments of members to determine their needs, including medical, mental health, substance use, financial situation, and housing. The ECM Lead Care Manager is responsible for organizing and facilitating outreach in the community and collaborating with partners to engage vulnerable populations, ensuring ECM services are provided using electronic health records (EHR).

The LCM is the primary point of contact for the member, the member’s family, the authorized representative (AR), the caregiver, other authorized support person(s) as appropriate, and the multidisciplinary care team providing care to the member. The LCM conducts a Comprehensive Assessment and develops a comprehensive Care Management Plan with input from the member and/or their parent, caregiver, guardian, and multidisciplinary care team, to ensure a whole-person approach is taken in identifying gaps in treatment or gaps in available and needed services.

Role and Responsibilities

  • Responsible for coordinating with those individuals and/or entities to ensure a seamless experience for Member and non-duplication of services.
  • Conduct outreach to Members as necessary for enrollment and engagement purposes
  • Serve as community liaison to external stakeholders representing El Centro de Amistad
  • Conduct a Comprehensive Assessment and, under the supervision of the Clinical Consultant, develop a comprehensive Care Management Plan with input from the Member and/or their parent, caregiver, guardian, and multidisciplinary care team.
  • Engage eligible members, primarily through in-person contact or the member’s preferred method of communication
  • Manages referrals and coordinates and follows up on needed services and supports
  • Organizes member care activities and maintains at least monthly contact with their providers to ensure coordination, including referrals to specialty mental health.
  • Oversee provision of ECM (Enhanced Care Management) services and implementation of the care plan
  • Offer services where the member lives, seeks care, or finds most easily accessible and within MCP guidelines
  • Connect Member to other social services and support the Member may need, including transportation.
  • Advocate on behalf of Members with health professionals
  • Use motivational interviewing, trauma-informed care, and harm reduction approaches
  • Coordinate with hospital staff on discharge plan
  • Accompany Member on office visits, as needed and according to MCP guidelines.
  • Monitor treatment adherence (including medication)
  • Provide health promotion and self-management training
  • Crisis Management and patient advocacy
  • Document Care management encounters in the Electronic Health Record (EHR) with the appropriate billing codes and internal tracking logs
  • Supports the members in managing their medical conditions.
  • Supports the members during discharge (i.e. hospital and other treatment facilities).
  • Attends required ECM trainings
  • Attend team, supervisory, and staff meetings.
  • Other duties as assigned

Qualifications and Education Requirements

  • Minimum of a bachelor’s degree in social sciences with a minimum of 1 year of Case Management experience. Previous experience in a healthcare setting is a plus.
  • Preferred: Master’s Degree level education or licensed waivered/licensed (i.e. AMFT, LMFT, ASW, LCSW)
  • Familiar with working with managed care plans and/or Medi-Cal
  • Must have experience managing a high client caseload
  • Strong understating of community outreach and peer support
  • Strong documentation, attention to detail, and ability to multitask and prioritize when needed
  • Familiarity with Electronic Health Record Systems
  • Able to travel to meet with members, attend professional meetings, conferences, as assigned
  • Have knowledge of chronic medical conditions, mental illness, substance use, homelessness.
  • Bilingual (English/Spanish) with excellent oral and written communication skills.
  • Must have great organizational skills, be detail oriented and a self-starter with the ability to multitask.
  • The ideal candidate should be personable and have experience in the social services field with familiarity in the mental health field.
  • Candidate needs to be motivated and able to communicate effectively, and efficiently, both verbally and in written format.
  • Strong computer literacy including Microsoft Word, Excel, Access, Adobe PDF, Outlook, Google Drive, Internet, and other standard equipment.
  • Must clear HIPAA certification, Department of Justice, and TB clearance.
  • Must have a reliable vehicle/access to transportation, valid California driver’s license, and be responsible for all liability on vehicle.

Candidates must be able to work effectively with their team and Manager, demonstrate adaptability, and can accomplish program goals and objectives. Candidates should demonstrate empathy, courtesy, interest, and a willingness to adhere to safety rules, as well as computer, email, Internet, and dress code policies. Must have valid California driver’s license and use of personal vehicle that has liability car insurance. Candidates will receive their required BBS supervisory ratio for client services provided.

Staff must be able and willing to work with clients who have special needs and/or aggressive behaviors (i.e., biting, kicking, hitting, emotional outburst, etc.). Staff must have behavior management strategies and techniques relating to clients experiencing behavioral difficulties, positive behavioral interventions and applied behavioral analysis.
