Job Summary

Summary

Responsible for completing intake interviews; determining need for medical and psycho-social evaluations; reviewing therapist and physician evaluations, care objectives, and plans; coordinating services being provided; advocating for needed services and entitlements.



Essential Functions
Complete psychosocial assessments covering psychological, social and environmental concerns, including evaluation of mental status.

Review daily referrals and screenings of clients.

Audit case file on a regular basis for completeness and accuracy.

Assist in discharge planning as required

Serves and protects the hospital community by adhering to professional standards, hospital care policies and procedures, federal, state, and local requirements, and Joint Commission on Accreditation of Healthcare Organizations (JCAHO) standards.

Qualifications

Education

Master's Degree Social Work required or

Master's Degree Related Field of Study preferred or

Master's Degree Mental Health required

Licenses and Credentials

Social Worker, Licensed Clinical [LCSW - State License] preferred

Social Worker, Licensed Independent Clinical [LICSW - State License] preferred

Occupational Therapist [State License] preferred

Physical Therapist [State License] preferred

Certified Rehabilitation Counselor [CRC] preferred

Licensed Mental Health Counselor [State License] preferred

Licensed Rehabilitation Counselor [Massachusetts] preferred

Respiratory Therapist [Massachusetts] preferred

Experience

case management, utilization review, or discharge planning experience 2-3 years preferred

Knowledge, Skills & Abilities

Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times.

Using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems.

Ability to establish strong rapport and relationships with patients and staff.

Proficient in Microsoft Office and industry related software programs.

Identifying complex problems and reviewing related information to develop and evaluate options and implement solutions.

Ability to maintain client and staff confidentiality.

Understanding of diagnostic criteria for dual conditions and the ability to conceptualize modalities and placement criteria within the continuum of care.

Knowledge of Healthcare and Managed Care preferred.
