VPV Care Coordinator (VPVCC) (Full-time, 40 hours) Exempt

BUILD, Inc. (Broader Urban Involvement and Leadership Development) is presently seeking a committed, self-motivated, and innovative individual to join our team. This position will be dedicated to the Violence Prevention Villages (VPV) project that aims to provide positive, proactive and preventive upstream support and services to youth Pre k – 8 and their households. BUILD partners with schools to improve behavioral, social/emotional and school outcomes for students, and we engage the nonprofit ecosystem to support household wellbeing and stability. We follow and support youth and households if they are willing, and aim to improve long term well-being, educational attainment and economic stability across generations.

The Care Coordinator will be responsible for conducting assessments, tracking progress and identifying and securing appropriate resources to support youth aged 3 – 15 and all members of their households. The Care Coordinator will work with a team of direct service providers as well as leverage a cadre of partner agencies to address the needs of selected youth and households. They will facilitate care coordination meetings for some cases to ensure maximum efficiency and strategic application of resources. The Care Coordinator will also be responsible for working with the BUILD organization to develop additional partner agency relationships as needed to meet needs in the youth and households they serve. They will also utilize assessment tools for initial state assessment and ongoing progress monitoring and use available technical tools to track centralized data. Work will primarily take place at assigned neighborhood school(s), as well as at the BUILD campus, in the homes and other locations as needed. The Care Coordinator reports directly to the Manager of Violence Prevention Villages.

Responsibilities Includes But Not Limited To

Assessment, Monitoring and Service Allocation:

  • Conduct initial assessments of youth, their household, and all members of their households (as needed) to identify service opportunities and utilize that data to allocate services consistent with the VPV program model
  • Conduct ongoing progress monitoring assessment to adjust service allocations, inputs and providers as needed to sustain or increase progress, or taper off services if goals are achieved
  • Ensure appropriate type and levels of service provision
  • In collaboration with the VPV Manager, conduct CARE team/Case review team meetings to engage BUILD staff, relevant school staff and others as appropriate to conduct shared assessment and planning for maximum benefit to the youth and household/household members
  • Identify gaps in service array and work with VPV Manager and/or other BUILD staff to build new partners to address those gaps

Documentation And Competencies

  • Contributes to client related data systems for record keeping and reporting purposes
  • Contributes to reports as required per grant
  • Ensure the timely flow of case management activities to meet program service plan goals
  • Utilizes BUILD Universal Referral form to document and connect families to other BUILD services as needed
  • Operates professionally in the field on a flexible schedule, which may include evenings and weekends
  • Completes all assessments, service plans, case notes and any other significant program data within a timely manner
  • Maintains all forms required, including case notes, client service plans, referral forms, information releases related to health or disabling conditions, health information, and any other information necessary in program participants file utilizing all procedures related to maintaining confidentiality at all times

Communication And Relationship Management

  • Establishes a regular schedule of classroom and/or home visits, contacts, and follow-ups to encourage the building of relationships with participants and ensure completion of service plans
  • Maintains active, open and consistent communication with school counselor and other appropriate school staff to allow for aligned services to youth and their household members
  • Represents BUILD to broaden community awareness and support, interpreting the agency’s programs and services at program-related community, coalition and committee meetings as needed
  • Attends departmental and agency-wide staff meetings, as scheduled and agency-wide events, as required
  • Maintains knowledge current in the field in way of participation of coursework, seminars and/or workshops

Other

  • Participate in agency-wide fundraising events
  • Participate in agency outreach and recruitment events, including evenings and weekends
  • Participate in civic engagement activities that advance the mission of BUILD, Inc.
  • Perform other duties as assigned

Requirements

  • Minimum three years of experience working with inner-city youth in a human service setting, trauma-informed care, social service setting, or related field
  • Strong organizational skills
  • Must demonstrate excellent interpersonal, verbal and written communication skills
  • An automobile is required along with current insurance and a valid driver’s license
  • Maintain respectful and appropriate relationships with program participants, all levels of agency staff and the various publics of the agency, demonstrating sensitivity to all ethnic, racial, and economic backgrounds
  • Must be willing to travel as needed
  • Must be passionate about youth development work
  • Must successfully pass extensive background check
  • Must be able to provide proof of vaccination against COVID-19
