Job Title: Home Health Intake CoordinatorJob Summary:
The Home Health Intake Coordinator is responsible for managing the intake process of patients requiring home health services. This includes ensuring that all necessary documentation is collected, processing referrals, coordinating with healthcare providers, and assisting with scheduling home visits. The Intake Coordinator plays a key role in delivering high-quality patient care by ensuring a smooth, efficient, and accurate intake process.
Key Responsibilities:
- Patient Intake Management:
- Receive and review patient referrals from physicians, hospitals, or other healthcare providers.
- Verify patient eligibility for home health services based on insurance and other criteria.
- Collect, review, and process patient information, including personal details, medical history, and insurance information.
- Ensure all required paperwork is completed and accurate.
- Coordination with Healthcare Providers:
- Communicate with physicians, medical staff, and other providers to gather necessary information for patient care plans.
- Ensure that all orders, including physician certifications and signed documents, are received and accurately processed.
- Follow up with healthcare professionals to ensure timely receipt of any missing documents.
- Scheduling and Coordination:
- Schedule initial home health assessments and visits by field staff (e.g., nurses, physical therapists).
- Coordinate with clinical teams to ensure timely and accurate delivery of care services.
- Communicate with patients and their families to explain service details and confirm appointments.
- Patient Communication:
- Serve as the primary point of contact for patients and families during the intake process.
- Address patient questions or concerns regarding home health services, insurance coverage, and the care process.
- Provide information on available services and help set expectations for the home health care experience.
- Compliance and Documentation:
- Ensure compliance with all healthcare regulations, including HIPAA and patient confidentiality standards.
- Maintain accurate records of patient information and referrals in the organization's database.
- Ensure that all required documentation is submitted to the appropriate authorities in a timely manner.
- Problem Solving and Follow-Up:
- Address any issues or delays in the intake process promptly and escalate if needed.
- Follow up with patients, families, and clinical teams to resolve any outstanding issues and ensure smooth transitions into care.
- General Administrative Support:
- Provide administrative support to the home health care team.
- Assist with preparing reports, tracking referrals, and updating patient information.
- Ensure that intake-related policies and procedures are followed.
Qualifications:
- Education:
- High School Diploma or GED required.
- Associate's or Bachelor's degree in healthcare administration or related field preferred.
- Experience:
- Minimum of 1-2 years of experience in a healthcare setting, preferably in home health or intake coordination.
- Experience with patient scheduling and managing healthcare documentation is a plus.
- Skills:
- Strong organizational and multitasking skills.
- Excellent communication and interpersonal skills.
- Knowledge of medical terminology and healthcare documentation practices.
- Ability to work independently and as part of a team.
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
- Certifications:
- Certification in medical office management or home health coordination is a plus.
Work Environment:
- Typically office-based, but occasional field visits may be required to coordinate patient care.
- Requires attention to detail and the ability to manage a high volume of tasks efficiently.
Working Hours:
- Full-time position with flexible hours.
