Position: Full-Time Care Coordinator
About the Role: We are seeking a dedicated and detail-oriented Care Coordinator to join our team. In this role, you’ll play a key part in the administrative side of youth services, working closely with State entities and our Agency partners to process referrals and ensure all necessary authorizations are in place. You’ll collaborate with our Intake and Permanency departments, ensuring that services and support are seamlessly coordinated to help children thrive in their homes.

Your Daily Responsibilities
- Manage and process youth referral information.
- Ensure service authorizations are accurate and up to date.
- Coordinate with various departments to maintain service cohesion.
- Communicate effectively across multiple levels of supervision and agencies.
- A high school diploma or equivalent.
- A valid driver’s license and ability to meet agency underwriting standards.
- An eye for detail and strong organizational skills.
- The ability to stay busy and manage multiple tasks.
- A passion for helping children and families succeed.
