Title: Intensive Care Manager – Dream Free Program - Statewide Human Trafficking Services (youth) - The region covered by this position is the northernmost counties of NJ such as Bergen, Essex, Hudson, Morris, Passaic, Sussex, and Warren. This position involves spending the majority of the time working in the field; it is not a hybrid remote, or office based.

Title: Intensive Care Manager – Full Time - Dream Free Program

Purpose: To provide coordinated, trauma-informed, holistic direct services to individuals of all races, nationalities, cultures, spiritual practices, abilities, sexual orientations, gender identities, and gender role.

Duties:

1. Responsible for intake assessment, development, and implementation of case plan based and ongoing care management services

2. Responsible for appropriate system linkages and referrals to support case plan

3. Responsible for monitoring and tracking service utilization.

4. Responsible for evaluation of services and outcome measures

5. Responsible for providing direct client services including but not limited to case management, counseling, advocacy, and group facilitation

6. Responsible for crisis responses

7. Responsible to assist in providing training to other social services organizations and the community

8. Responsible for the collection and maintenance of client records and service data

9. Responsible for other duties as assigned by the Program’s Team Lead and Director

Accountability: The Intensive Care Manager is responsible to the Program’s Team Lead and Director, and the client population for the timely, professional, and effective performance of all job duties.

Qualifications:

1. Bachelor’s Degree in Social Work or a related field, or three years of related field experience and transferable skills

2. Experience in working individually with the client population or experience with a similar population that is transferable

3. Experience in direct client service delivery, and in working with people from diverse racial, ethnic, and socioeconomic backgrounds

4. Grounded in anti-oppressive practice

5. Honors the value of working in diverse teams

6. Ability to provide responsive communication

7. Ability to speak a second language valued – Spanish preferred

8. Ability to network and advocate within and outside the Agency on behalf of the client population

9. Willingness to utilize, share and/or develop technology skills

10. Valid New Jersey driver’s license and insurability through AVANZAR’s insurance carrier
