General Description

  • Responsible for supportive counseling and providing training in independent living skills.

Duties and Responsibilities

  • Participates in the planning and evaluations of individual service plans and monitors plan objectives.
  • Write progress notes on service plan monthly or more often as needed.
  • Trains and assists residents in the activities of daily living.
  • Monitors medication.
  • Responsible for accompanying residents and providing needed transportation for recreational and social activities.
  • Provide counseling, behavioral intervention and crisis management services.
  • Assist in day-to-day management of the community residence.
  • Responsible for administration of residence in the absence of the Senior Counselor.
  • Notifies the appropriate supervisory personnel of any medical, psychiatric, or programmatic emergency. (See Functional Program)
  • Responsible for other related activities.
  • Provides case management and follow-up services.
  • Provides transportation, as needed, for shopping, medical appointments, etc.
  • Learns and adheres to all agency policies and procedures on an ongoing basis.
  • Accepts and follows through on all other duties and responsibilities as assigned.

Qualification and Competencies

● High School Diploma and five years’ experience in the Human Service field or bachelor’s degree in human service field

● Demonstrates organizational and communication skills which would enable effective interaction with agency staff, consumers/clients/families, and the community.

● Critical thinking and sound judgment skills are necessary.

Physical Demand and Working Conditions

  • Adheres to HIPAA Policies & Procedures/Compliance Plan.
  • Valid Driver’s license & available car
  • Submit Copy of Current Car Insurance
  • Required to take all mandatory trainings

Job Type: Full-time

Pay: $19.70 per hour

Schedule:

  • 8 hour shift
