Hospice & Palliative Care of Iredell County (HPCIC) is seeking a compassionate, mission-driven Palliative Care Social Worker to join our exceptional interdisciplinary team. Key Responsibilities

  • Provide emotional support, counseling, and psychosocial assessments to palliative care patients and their families
  • Conduct visits in a variety of settings including patient homes, facilities, and by phone
  • Collaborate closely with the palliative care medical team to develop care plans aligned with patient goals
  • Advocate for patients and families navigating complex healthcare decisions and community resources
  • Maintain timely, thorough documentation and contribute to interdisciplinary team discussions

Requirements

  • Master’s Degree in Social Work (MSW) required
  • Licensed Clinical Social Worker (LCSW) or Licensed Clinical Social Worker Associate (LCSW-A) preferred
  • Supervision for LCSW-A may be provided as part of this role
  • Passion for serious illness care and a strong commitment to holistic, person-centered support
  • Excellent communication and interpersonal skills
