Hospital Social Worker Highlights:
??? Schedule: 8:00 AM ??? 4:00 PM with a weekend requirement
??? Pay Range: $55-60 Hourly

Hospital Social Worker Responsibilities:
??? Provides therapeutic intervention and social work services to patients and their families to enhance integrated and uninterrupted care in the hospital and community.
??? Assesses the patient???s psychosocial situation in relation to medical needs and plans appropriate interventions.
??? Interviews patients, family members, and/or significant others to assess social work needs and social determinants of health.
??? Participates in multidisciplinary rounds and provides psycho/social context.
??? Functions as a resource and advocate for advanced care planning discussions.
??? Provides psychosocial support during adjustment to illness and life changes.
??? Coordinates post-hospital care including long-term, rehab, SNF, dialysis, hospice, or homelessness resources.
??? Documents psycho-social assessments, family interventions, and discharge plans in accordance with policies and regulatory requirements.
??? Develops and maintains a network of social service and government agency resources.
??? Participates in program planning and evaluation in alignment with hospital mission and goals.

Hospital Social Worker Qualifications:
??? Graduate of an accredited school of Social Work (MSW required)
??? Previous medical hospital social work experience strongly preferred
??? Excellent communication, negotiation, and conflict resolution skills
??? Knowledge of clinical practice trends, healthcare informatics, and regulatory standards
??? Expertise in rapid cycle change and clinical performance improvement
??? Physical capability to perform essential job functions, including standing, walking, lifting, and coordination
??? OMS will assess physical abilities and health/safety requirements
