Job Overview
The Intake Coordinator/Targeted Case Manager manages the intake and admission process for new clients, including conducting assessments and developing individualized service plans. This position provides case management services-planning, coordination, advocacy, and monitoring-to ensure that clients receive appropriate care and remain in the least restrictive environment possible. You will work closely with families, community partners, and service providers to support client wellness and stability.
key Responsibilities:
- Conduct face-to-face interviews with clients and/or their primary parent or legal guardian.
- Develop and implement individualized service plans tailored to client needs.
- Link clients with appropriate resources and advocate for service access.
- Conduct in-person home, school, and community visits; reliable transportation is required.
- Maintain strict client confidentiality and ensure compliance with agency privacy policy, Medicaid, and HIPAA standards.
- Collaborate with residential programs, DCF, DJJ, and other agencies for continuity of care.
- Monitor clients while in-patient or crisis stabilization units (CSU) ensure timely assignments of services within 48 hours.
- Attend required supervision, team meetings, all staff meetings, and professional trainings.
- Maintain or exceed agency monthly productivity minimum standard of 120 billable services hours.
- Conduct regular audits of documentation to ensure compliance with clinic policies and healthcare regulations.
- Completion of assessment and intake within 14 days of referral.

Pay: $18.00 per hour
