Job Summary
We are seeking a dedicated and compassionate LPN or RN for our Chronic Care Management Manager position on our healthcare team. The Care Manager will play a crucial role in coordinating patient care, ensuring that individuals receive comprehensive and effective services tailored to their unique needs. This position requires a strong understanding of patient care processes, utilization management, and the ability to work collaboratively with healthcare professionals. The ideal candidate will have experience in cardiology and geriatrics. This role will operate approximately 85% remote and 15% in office as needed.

Responsibilities

• Coordinate and manage patient care plans to ensure optimal health outcomes.
• Collaborate with healthcare providers, including physicians and specialists, to facilitate comprehensive care.
• Conduct assessments of patients' medical histories and current health status.
• Monitor patient progress and adjust care plans as necessary based on ongoing evaluations.
• Provide education and support to patients and their families regarding treatment options and health management.
• Advocate for patients’ needs within the healthcare system, ensuring access to necessary services.
• Maintain accurate documentation of patient interactions and care plans in compliance with regulatory standards.

Skills

• Strong background in patient care with a focus on quality service delivery.
• Proficiency in utilization management practices to optimize resource use.
• In-depth knowledge of physiology relevant to patient assessments and interventions.
• Familiarity with cardiology, primary care practices and geriatric patient needs.
• Understanding of behavioral health issues and their impact on overall wellness.

Job Type: Full-time

Pay: $30.00 - $35.00 per hour

Expected hours: 40 per week
