Requirements:
Degree in Social Work
Previous Long Term Care experience as a Social worker
Strong discharge planning skills.
Candidate must have excellent organizational and communication (written and oral) skills.
Ability to handle multiple priorities efficiently.
Strong Customer Service Skills.
Proficient in MDS 3.0 required.
SigmaCare EMR knowledge preferred.
