Job Summary:

As a Nurse Care Manager, you'll be responsible for providing comprehensive care management services, including assessing client needs, developing treatment plans, and coordinating with healthcare providers. Key Responsibilities:

Develop wellness and prevention initiatives to promote client health and wellbeing
Facilitate health education groups using advanced knowledge of complex medical conditions
Collaborate with treatment team members to develop and implement individualized treatment plans
Manage hospital and medical provider admissions and discharges for clients
Train patients on complex health conditions, medical diagnosis, treatment, and medications
Implement and utilize technology programs and initiatives to improve patient outcomes
Monitor and report performance measures and outcomes using sophisticated information systems

Education, Experience, and/or Credential Qualifications:

Active RN license
Experience working with the IDD population preferred
Knowledge of chronic disease processes in children and adults
Excellent written and verbal communication skills

Additional Qualifications:

Current driver's license, acceptable driving record, and current auto insurance
Successful completion of background check, including criminal record, driving record, abuse/neglect, and fingerprint check

Physical Requirements:

Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull objects
Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift
Sedentary work involves sitting most of the time, with occasional walking or standing for brief periods
