Reliant Care Managment Company has a rewarding opportunity for an experienced Care Plan Coordinator. Position will involve management in accordance with government regulations and company policies, coordinating the care plan process with focus on maintaining excellent care for the residents. Requirements include an unencumbered Missouri RN or LPN license with preferably 2-5 years experience in long-term care, knowledge of Medicare regulations, and a commitment to Total Quality Management. Working knowledge of Case Mix Index, Quality Measures tracking, QRP, Value Based Purchasing.

Responsibilities

• Conduct comprehensive assessments and evaluations of patient care needs, ensuring compliance with guidelines.
• Develop and implement individualized care plans in collaboration with interdisciplinary teams.
• Perform utilization reviews to ensure appropriate resource allocation and adherence to Medicare regulations.
• Maintain accurate medical documentation and records in EHR systems, ensuring HIPAA compliance.
• Provide guidance and support to nursing staff regarding patient care and documentation practices.
