Referral Intake Coordinator – Home Health

Location: Tacoma, WA
Schedule: Full-time, Monday to Friday, 8:00 am to 5:00 pm

Puget Sound Home Health & Hospice is seeking a detail-oriented and collaborative Referral Intake Coordinator to join our team.

This pivotal role is responsible for the frontline screening of incoming referrals to determine medical necessity and eligibility for home health services before they are approved for the intake team.

What Do We Require of You?

• **At least 2 years of intake experience, specifically with AIDA hospital referrals, Careport/Wellsky, Kaiser, Novari, and other similar accounts and platforms. This is a must have!**
• Experience with Home Care Home Base is strongly preferred
• Demonstrated cooperation and excellent customer service skills
• Ability to multi-task and stay on task in a fast-paced office
• Excellent team-oriented, verbal, and written communication skills

Key Responsibilities:

• Review and process high volume of incoming referrals from multiple platforms (AIDA, CarePort/WellSky, Kaiser, Novari, etc.)
• Pre-Screen referral documentation to assess medical necessity and eligibility for home health services
• Collaborate with the intake, clinical, and business development teams to ensure smooth handoffs
• Attend weekly referral and marketing team meetings and contribute to process improvements
• Maintain accurate records and report on referral trends or red flags
• Ensure compliance with contracts and admission policies
• Assist in identifying unmet needs and potential service expansion opportunities
• Promote a positive image of Alpha Home Health & Hospice in the community
• Participate in performance improvement and strategic planning initiatives
