Coverage Area: Washtenaw County

Our licensed clinical social workers perform initial patient assessments to identify psychological, emotional and safety needs for each of our home care patients. They develop and implement individualized plans of care in collaboration with the patient's physician and our multi-disciplinary team. They assess and determine patient/family needs and contact appropriate community resources. Our social workers maintain knowledge of federal, state and local laws and regulations and participate in quality improvement processes.

Minimum qualifications

  • Master's Degree in Social Work.

  • One year social work experience in a healthcare setting; home care/hospice experience preferred.

  • FULL Licensure as a Social Worker in the State of Michigan
