The Behavioral Wellness Center at Girard is a leader in providing comprehensive behavioral health and substance use disorder services in the Philadelphia region. As part of the innovative Philadelphia Model - Addressing Serious Mental Illness (SMI) in Long-Term Care (LTC) - we deliver structured, therapeutic residential care for frail adults with complex behavioral health needs. Position Summary
We are seeking a dedicated Social Worker to join our Behavioral Medicine team. This role is central to helping patients navigate mental health and substance use recovery by offering case management, discharge planning, resource coordination, and clinical counseling.
Available Shifts
- Full-Time Day Shift: 40 hours/week
- Part-Time Day Shift: 16 hours/week (Every Other Weekend)
- Complete psychosocial and mental health assessments
- Develop and implement treatment and discharge plans
- Provide counseling, crisis intervention, and advocacy
- Refer patients to community resources (housing, healthcare, recovery programs)
- Participate in interdisciplinary care planning and team meetings
- Maintain timely and accurate documentation in the EHR system
- Respond to crises, including conducting suicide risk assessments
Education & Licensure:
- Masters in social work (MSW) required
- LSW or LCSW preferred
- CPR certification preferred
- At least 1 year in behavioral health, substance use, or mental health settings
- Inpatient psychiatric or addiction recovery experience is a plus
- Knowledge of trauma-informed care, CBT, Motivational Interviewing, and harm reduction
- Strong interpersonal and clinical documentation skills
- Proficient in EHR systems and Microsoft Office
- Ability to thrive in a multidisciplinary, patient-centered team
- Direct interaction with clients in inpatient, outpatient, and long-term care settings
- High-emotional intensity; must manage crisis situations calmly
