JOB SUMMARY

Under the direction of the Director of Case Management, oversees daily operations and supervision of the Inpatient and Emergency Departments Social Work Team at all NCH campuses. Promotes patient/family focused care as the highest priority and strives to meet customer expectations for quality and patient satisfaction. Utilizes performance management expertise to facilitate the attainment of goals and holds staff accountable to goals, and adherence to policies/protocols.

ESSENTIAL DUTIES AND RESPONSIBILITIES

•Collaborate with case management leadership team to develop and implement organizational case management strategies with a focus on patient experience.
•Oversee a team of dedicated social workers ensuring effective program delivery.
•Ensures compliance with regulatory standards.
•Recruits, trains, and mentors social work professionals.
•Completes personnel functions including staffing, performance reviews and corrective action.
•Fosters a culture of continuous learning and growth.
•Supports staff by identification and utilization of resources.
•Implements discipline based best practice guidelines.
•Implements appropriate monitoring techniques to ensure documentation is completed per department guidelines.
•Holds regular staff meetings encouraging staff to share and participate in organizational and department quality improvement.
•Ensures staff participation in daily multidisciplinary rounds to discuss transition of care and any barriers to the discharge plan.
•Participates in weekly complex case review.
•Responsible for, but not limited to length of stay, avoidable days, prevention of readmissions, and discharge planning metrics.
•Recording/reporting of indigent fund spending.
•Assures timely, effective discharge planning that involves care team, payor, along with patient and their families
•Advocates and escalates any care delays.
•Participates in quality-of-care initiatives; reporting quality concerns to appropriate individuals, departments, and/or committees in a timely manner.
•Assists in the development and oversees implementation of care management policies and procedures
•Collaborates with Manager of RN Case Managers to ensure effective collaboration between disciplines
•Collaborates with utilization management team to ensure timely delivery of regulatory forms, appropriate level of care, long stay outliers, and patients exceeding geometric length of stay.

EDUCATION, EXPERIENCE AND QUALIFICATIONS
•Masters degree in Social Work (MSW) required.
•Minimum of 2 years of supervisory/lead/manager level of experience in hospital Case Management department is required.
•LCSW preferred; Case Management Certification preferred.
•Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows
