Pay: $33.82 - $50.73 / hour. Compensation is based on the candidate's relevant experience.

Summary:

This role is dedicated to delivering compassionate support as a Palliative Care Social Worker within the University of Colorado Hospital Palliative Care Service. You will collaborate with patients in various settings, including inpatient, emergency department, and clinics, to facilitate crucial discussions around care goals, advance planning, and transitions. Your expertise will help assess patients and families, arrange necessary interventions, and provide palliative care-focused strategies addressing emotional, social, and environmental needs, alongside grief support.

Responsibilities:

  • Offer specialized psychosocial support to palliative care patients through direct counseling and collaborative visits, with a commitment to cultural sensitivity.
  • Conduct and document thorough psychosocial assessments for palliative care patients.
  • Work closely with fellow social work team members to enhance support for patients in the palliative care service.
  • Assist in facilitating patient-family meetings to set values-based goals and care plans.
  • Identify and coordinate relevant outpatient services for palliative care patients.
  • Engage in educational and outreach services that support the social service-palliative care relationship.
  • Utilize critical thinking and decisive judgment while working with limited supervision in a fast-paced environment.

Requirements:

  • Master's degree in Social Work.
  • State licensure as a Licensed Clinical Social Worker (LCSW).

Preferred:

  • Willingness to pursue Palliative Care certification within two years.
  • At least two years of clinical experience in a hospital setting; experience in palliative care is strongly preferred.
