About Job

As Case Manager provides evaluation, consultation and intervention with children, youth and their families involved in Challenge Academy, Alternative Education Program and other aspects within the Juvenile Justice System, in the Encircle program. Also participates in conducting psychosocial programming with this population as part of a clinical team composed of School, Juvenile Probation, Truancy Court and Mental Health staff in an effort to reduce the recidivism rate from transitioning back to home campuses.

EXAMPLES OF WORK PERFORMED::

  • Works with youth and their families in mental health, home or office settings.
  • Provides case management and some skills training with children and families.
  • Makes assessment and psychosocial evaluations and recommendations.
  • Refers families of clients to appropriate social welfare and health agencies to obtain assistance.
  • Performs Related Work as Required.


Knowledge, Skills, and Essential Functions

  • Excellent written communication and computer keyboarding for inputting clinical documentation
  • Excellent verbal communication with consumers, staff, and community providers
  • Time management required to meet service goals
  • Local travel to consumer homes, school, and other community settings
  • Stooping, bending, sitting, standing, keyboarding for prolonged periods, and lifting 30 lbs.
  • Ability to be CPR and PMAB certified
  • Valid driver's license, liability insurance, and reliable transportation required. 50% of time will be spent traveling locally.
  • Knowledge of emotional problems and treatment approaches.
  • Ability to establish and maintain positive working relationships with consumers and colleagues.
  • Good work habits and organizational skills.
  • Level of general health permitting stooping, bending, climbing stairs and participating in moderately active recreational activities.

Education and Experience

  • Bachelor's or Master's in social work, psychology or related field with ability to obtain Qualified Mental Health Professional credentialing
  • 1 year of experience of case management or mental health preferred
  • Clinical documentation experience is needed.
  • Bilingual preferred

Hours: M-F 8AM-5PM with other as needed:
