About Job

Under the supervision of the IDD Supervisor of Local Authority Services and within federal, state, and local guidelines, performs routine work meeting the needs of individuals with developmental disabilities. Work involves assessing and determining individual resident needs and developing appropriate programs.

Duties include screening, assessing, authorizing and coordinating eligibility and access to IDD Services for individuals with developmental disabilities. The position will also provide back-up to the Continuity of Care Coordinator as requested. Works under moderate supervision, with limited latitude for the use of initiative and independent judgment.

Education, Training and Experience (including licensure and certification)

Graduation from an accredited four-year college or university with major course work in human services or a related field and one year of MH/IDD experience preferred. Experience and education may be substituted for one another.

REGISTRATION, CERTIFICATION, LICENSURE OR OTHER QUALIFICATIONS:

  • All job specific training must be completed within 30 days of hire.
  • Must maintain a valid Texas driver's license, auto liability insurance and a driving record acceptable to the Centers insurance requirements.
  • Successful completion of all assigned staff development courses required.

Knowledge, Skills and Abilities

  • Knowledge of rehabilitation programs for individuals with developmental disabilities.
  • Skill in the operation of computers and in applicable computer software.
  • Ability to collect and analyze data, to design therapeutic programs, to prepare and maintain records, and to communicate effectively.

Essential Duties

Screens non-emergency requests for services within 24 business hours and urgent needs immediately to determine potential eligibility and need. Reviews Explanation of Services with 100% of new inquiries.

  • Ask appropriate screening questions to gauge the caller's service needs and provide necessary information to caller in a timely manner.

Schedules Intakes and Eligibility Determinations as needed.

  • When appropriate, schedule Intake and DID appointments with available Psychologists for individuals needing Eligibility Determinations in a timely manner.

Ensure CARE registration for all new clients.

For all new clients, fill out a CARE registration form describing the type of services requested, and submit to the LA Team Leader for entering.

Perform Intakes for new clients.

Complete and review all necessary Intake paperwork with all scheduled clients.

Ensure intake procedures are completed for all new clients.

Complete intake notes, initial PDP, fee assessment fact sheet, and related CARE forms and submit them to the appropriate staff.

Ensure eligibility procedures are completed for all new clients.

Once the DID report is completed, make appropriate copies to be submitted to Medical Records and mail the eligibility determination letter to the client or family.

Refer necessary clients for GR Service Coordination.

  • When GR services are needed, correctly complete the GR Service Coordination referral form with the client's PDP and DID attached and submit to the GR Service Coordination Supervisor for assignment.

Completes the process for HCS diversion slots as required.

  • When necessary, complete and submit all required HCS diversion slot paperwork in a timely manner.
  • Facilitates SSLC referrals to the CLOIP Director upon request 100% of the time.
  • When SSLC placement is requested, consult with the CLOIP Director and provide necessary contact information.
  • Completes authorization process for ICF placement.
  • When placement is needed, complete referral letters and fax to appropriate facilities with the DID and ICAP information.
  • Completes HCS/TxHmL enrollments per contract requirements and Center policies and procedures as assigned.
  • When requested, perform HCS/TxHmL enrollment duties such as scheduling meetings, meets with clients, completing paperwork, and activating client services in a timely manner.

Coordinates referrals and services for individuals not assigned to Service Coordination as needed.

  • For clients not receiving GR services for any reason, provide referrals/resources for outside services (e.g. DARS, STAR PLUS Waiver, CLASS)

Receives, manages, and tracks CFC referrals from MCOs through the portal.

Contact clients referred by the MCO and schedule intake/DID appointments.

Completes ID/RC to determine LOC for CFC.

Following the completion of the eligibility determination, complete the ID/RC paperwork and fax to the DADS CFC department. Make sure to send the LOC fax sheet, ID/RC, DID, and ICAP scoring sheet.

Completes the CFC Functional Assessment and develops and submits recommended service plans to the MCOs for individuals in CFC.

Once the LOC is approved by DADS, the functional assessment must be completed with the client/family and then submitted to the appropriate MCO for review and approval.

o Renews the CFC assessment annually thereafter.

Assumes a CFC caseload once individuals begin to get enrolled and provides monthly Service Coordination.

Perform all Service Coordination duties for CFC clients once they begin receiving services in a timely manner.

Make all necessary CFC referrals.

When a client is under the age of 22, refer the client for CFC by calling 1-888-276-0702 and provide all necessary information about the client.

Provides back-up assistance to the LA Team Leader as needed.

  • When requested, provide immediate assistance to the LA Team Leader (e.g. providing resources, provide scheduling information, speaking with parents/clients).

Provides crisis intervention with minimal supervision.

During a crisis situation, provide any assistance and/or crisis intervention.

Participates in program planning and interagency coordination of Medicaid service programs.

Participate in CRCG when held.

Accurately completes CARE forms and submits for data entry in a timely manner.

Complete all necessary CARE forms related to each client and submit to LA Team Leader.

Assume a temporary GR caseload when deemed necessary by a supervisor.

  • Assist with making monthly face to face contacts and completing/renewing appropriate paperwork.
  • Maintains updated resource book and referral information.
  • Keep resource books organized and up to date and add any new resources that are acquired. Keep all resource packets in supply and updated.
  • Participates in Medicaid Administrative Claim Time Studies as scheduled. Performs verbal PMAB and CPR as circumstances require. Attends agency networking meetings and agency fairs for the purpose of outreach as requested.
  • Perform necessary tasks and attend agency events as requested or needed.
  • Performs additional duties as deemed necessary by department supervisor within designated timeframes.
  • Complete all necessary tasks requested by a supervisor in a timely manner.
