Position Summary
The RN Care Manager is responsible for completing comprehensive assessments upon admission, coordinating multidisciplinary care, and managing discharge planning and utilization review (UR) activities. This role ensures patients receive appropriate support services to promote safety, reduce readmissions, and enhance satisfaction.
Responsibilities
- Conduct comprehensive assessments and reassess patient needs throughout the hospital stay
- Coordinate multidisciplinary care and discharge planning for diverse patient populations
- Educate patients and caregivers on discharge plans and aftercare
- Perform utilization review and complete documentation per department policies
- Collaborate with healthcare team members and external providers
- Participate in performance improvement and quality initiatives
- Contribute to length-of-stay and readmission reduction efforts
- Support transitions of care, especially for high-utilizer and geriatric patients
Requirements
- BSN required (or must obtain by 12/31/21 if hired prior to 1/1/18)
- NYS Registered Nurse (RN) license required
- PRI Certification (within 3 months of hire)
- Care Management Certification (e.g., CMC, CCM) preferred
- Experience in ED, med/surg, or critical care preferred
- Prior case management and InterQual/Milliman experience preferred
- Computer proficiency and strong communication skills essential
- Familiarity with payer policies and healthcare regulations preferred
Job Types: Full-time, Part-time
Pay: $119,971.99 - $153,900.24 per year
