Job Overview

We are currently seeking a Bilingual (Spanish-English) Care Coordinator to join our Remote Patient Monitoring (RPM) team. This role is essential to supporting patients with chronic health conditions by monitoring their vitals remotely, ensuring they stay on track with their care plans, and promoting overall wellness through regular communication and care coordination.

Remote Patient Monitoring (RPM) is a technology-driven program that enables healthcare providers to monitor patients’ vital signs and health data, such as blood pressure, glucose levels, and weight, from their homes. The goal is to reduce hospital visits, detect complications early, and provide patients with continuous, personalized support.

Responsibilities

• Monitor incoming patient data and escalate abnormal findings to appropriate clinical staff.
• Conduct regular outreach to patients to ensure device compliance and provide health-related support.
• .Track patient adherence to care plans, medications, and health goals, offering encouragement and interventions as needed.

Experience

Minimum 1 year of clinical experience as a Medical Assistant or CMA

Bilingual in Spanish and English (Required)

Strong customer service and patient engagement abilities

Job Type: Full-time

Pay: $19.00 - $21.00 per hour

Expected hours: 40 per week
