Our efforts are focused on serving with housing, wrap-around services, and focused financial assistance. We also serve hot meals and meal boxes. Job Title: Temporary Care Coordinator

Reports To: Director of Programs

Status: Temporary Full-Time/Non-Exempt

Summary:

The Care Coordinator will assess participant needs and abilities to engage, answer questions, and make sure participants get to the correct service provider, class, or group to fit their needs. Participants often will arrive in varying stages of trauma due to homelessness or other traumatic life events. There will be components of mentoring, coaching, and kindness opportunities.

Job Duties:

Case Management

  • Clear, respectful, and frequent communication with volunteers, CBO’s and fellow staff
  • Greet callers via the phone
  • Maintain assigned Case Load
  • Keep accurate and up to date case notes
  • Maintain frequent communication with CBO’s connected to their case load
  • Attend all required staff meetings, trainings, and monthly all staff meetings
  • Attend tabling’s at community events quarterly
  • May be required to attend occasional fund-raising/community events
  • Required to learn, participate, and maintain external data systems (EX: TLR SharePoint, HMIS, and Coordinated Entry)
  • Follow established procedures and direction from Center Supervisor at the center
  • Give participants the support, encouragement, and referrals they need
  • Support the Center Supervisor with always maintaining an operational flow at the center
  • Support the Center Supervisor with covering critical areas of the center due to call outs or short staffing
  • Case management of housing residents at the Center
  • Meet all required deadlines

Center Organization and Safety

  • Assist the Center team (staff and volunteers) to maintain order and cleanliness
  • Work with the Center team to make sure everyone is emotional and physically safe while at The Center
  • Identify and communicate any additional needed services, protocols or trainings needed for The Center to strengthen and improve the quality of the experience for participants
  • Help in conducting enrollments, assessments, care coordination of your assigned participants and residents
  • Help plan celebrations for the individual and collective achievements of The Life Center Community
  • Work in concert with staff and volunteers to maintain a culture of positive support and kindness
  • Work in concert with the staff and volunteers to maintain the cleanliness and order of all classes, groups, and activities
  • Communicate with all staff and volunteers if there are any situations that may escalate or that seems to be potentially disruptive.
  • Work to deescalate any situations that might flare up into a negative exchange or situation
  • Other duties as assigned

Environment:

  • While performing the duties of this job, the employee works both indoors and outdoors
  • May be exposed to equipment vibrations, noises, and odors, wet and/or slippery surfaces.
  • May include traveling to meetings, conferences, and work assignments
  • The employee must be able to meet deadlines with severe time constraints.
  • The noise level in the work environment is usually moderate and often work in a shared space
  • Some evenings and weekends are required for community functions and events
  • Occasionally operate a motor vehicle

  • Must be able sit for extended periods

  • Frequently required to use a computer and phone
  • Prolonged periods walking or standing
  • Must be able to lift, carry, and place up to 40 pounds at a time
  • Specific vision abilities required by this job include close vision, depth perception, peripheral vision, and color vision
