Job Description
The Program Counselor assumes primary responsibility for case management of the assigned patients. Including but not limited to assessment, treatment planning, individual and group counseling, charting, discharge planning, liaison activity with significant others, communication as necessary with referral sources and third party payers, and didactic presentations as assigned.

Including but not limited to assessment, treatment planning, individual and group counseling, charting, discharge planning, liaison activity with significant others, communication as necessary with referral sources and third party payers, and didactic presentations as assigned.
Requirements:
- Must be licensed as an LCDC, LPC, LCSW or another Qualified Credentialed Counselor in Texas.
- A Bachelor or Master’s degree in a Human Services field from an accredited institution is preferred.
