RSS currently has an opening for a Care Manager within a new Critical Time Intervention (CTI) program in the Capital District. The qualified candidate will play an important role in an innovative team working in collaboration with the hospital system (Albany Medical Center).
This is an exciting opportunity for a Care Manager who is looking to transform community healthcare and making long lasting, positive changes in the lives of complex individuals.
The Care Manager’s role will involve outreach to individuals in the ED, on inpatient units and post-discharge. The Care Manager will work to connect individuals to resources and services necessary for long-term stabilization such as benefits, income, housing, treatment, transportation and other supports. Member choice, harm reduction, non-coercion, flexibility, and person-centered care are essential elements of the CTI program model and should be front and center of the care delivered by the Care Manager.
Schedule:
Sunday-Wednesday 10am-8pm
Qualifications:
- Bachelor’s degree or higher, preferable in psychology, social work, sociology, or related field or be a New York State Licensed Practical Nurse (LPN)
- Case Management work experience in a social service agency, preferable serving a behavioral health population
- Four years of past case management work experience may be considered in lieu of Bachelor’s degree
- Preference will be given to those who have lived experience
Essential Knowledge, Skills and Abilities:
- Experience working with complex and homeless and/or precariously housed populations preferred but not required
- Knowledge of mental health, substance use and related resources a plus
- Knowledge of counseling principles and methods for mental illness and substance use disorders
- Knowledge of treatment, rehabilitation, and community support programs as they relate to recipient/residents, families, and staff
- Ability to develop, evaluate, implement, and modify treatment intervention to meet the needs of individual recipients
- Ability to prepare accurate and timely reports
- Computer proficiency and good documentation skills
