As one of our Licensed Clinicians, you are responsible for the client customer experience in relation to the oversight of their initial evaluation, care/treatment plan and/or transition of care, step down care, and continuity of care as needed. This position reports to the Clinical Director and also collaborates with the Medical Director for medication management when applicable. Under general supervision, this position is responsible for ensuring seamless care for clients in our behavioral health outpatient program by acting as liaison between patients and other clinical care team members.

LPC/PLPC collaborates with a multidisciplinary team. It is the primary responsibility of the LPC/PLPC to ensure that the most appropriate quality care is provided to clients. Strong interpersonal skills, organization, flexibility, time-management, and can-do attitude will provide day-to-day success. The primary responsibility of the Independently licensed social worker is to provide individual therapy to adolescents, transitional age youth, or adults in our Outpatient Programs. It is also your responsibility to help diagnosis and/or refer as needed to our SUD IOP programming when deemed medically necessary. Therapist will assist clients using best practices of evidence-based techniques.

Requirements:

  • Already or able to be credentialed with Medicaid, Medicare, and Commercial Payors
  • Licensed Professional Counselor (LPC or PLPC)
  • Bachelor's Degree in Social work or related mental health field
  • Masters degree in a mental health related field

Qualifications:

  • Familiar with intake and doing Initial Evaluations
  • 3-4yrs min experience in a health-related setting (preferred)
  • CPR certified
  • Crisis Intervention Certification or proof of training
  • Suicide Prevention training preferred
  • Valid Driver’s License, in good standing
  • Professional liability insurance
  • Proficient with computers and EMR systems
  • Must have a personal computer and internet access
  • Valid NPI
  • Current CAQH profile and up to date attestation
  • Sound understanding of Evidence Based Practice (EBP)
  • Experience in Trauma, Addiction, SUD, Dual Diagnosis
  • Experience with Groups and SUD IOP (preferred)
  • Prior experience in a collaborative role with MDs/Psychiatrists
  • Excellent written & verbal communication in English (Spanish or any other language is a plus)
  • Adaptable to technology, including telehealth software, comfortable with video communication and the use of EHRs/EMRs
  • Responsive to texts, phone calls, and emails during business hours and/or designated work hours.
  • Solid PC skills, specifically using MS Word, Outlook, and Excel

Responsibilities:

  • Conduct comprehensive psycho-social histories, Intake Assessments, and initial evaluations; meeting all compliance requirements for charting and payor requirements
  • Chart individualized, behavioral & measurable treatment goals based on information in psychosocial assessment and psychiatric evaluation.
  • Document and maintain clinical review findings, actions and outcomes in accordance with policies, and regulatory and accreditation requirements
  • Provide regular individual therapy sessions using evidence based modalities and techniques
  • Experience with SAFETY plans and how to engage family and support systems during treatment with clients
  • Complete all compliance documentation, including but not limited to: Initial Evaluations; Plan of Care Reviews; Master Treatment Plans; Suicide and Crisis Assessments and Planning; Relapse Prevention, Urgent Coordination Notes; Mental Health Professional Progress Notes, and other documentation as required
  • Ensure that progress notes are performed on each client and charted within 24hrs
  • Coordinates efforts to provide mental health services in conjunction with other providers.
  • Collaborate with psychiatrist to ensure quality continuation of care and remind clients to schedule medication management appointments as needed based on the care plan
  • Periodically monitor psychiatric and mental health records, provides appropriate feedback, and, when necessary, takes corrective action
  • Coordinates with the Clinical supervisor, clinical director, medical director for client care relative to periodic consultation, annual review, medication management; and, other matters
  • Respond appropriately and in a timely manner to patient requests and/or inquires
  • Provide comprehensive education, instructions, and/or advocacy information designed to assist the patient and significant others to identify secure needed health care follow-up services.
  • Provide referring physicians and/or psychotherapists, counselors, and care team with weekly updates of patient’s compliance with and progress in treatment.
  • Adheres to HAMG’s Policies and Procedures.
  • Prepare and assist with audits when requested
  • Perform other duties as assigned.
  • Initiates and coordinates emergency hospitalization as required and coordination of follow up care
  • Maintains effective communication with clients, parents, guardians, and others involved in coordination of client care
  • Meets all regulatory and licensure requirements mandating availability for crisis intervention as needed
  • Maintains consistent reporting to immediate supervisor
  • Interfaces with departmental employees in the coordination of required documentation, communication, timelines, and other requirements for smooth scheduling and care planning for clients
  • Perform any other related duties as required or assigned
  • Provide feedback for QI/QA to include, but not limited to: coaching and development, rewards and recognition, performance management. Plan, organize, develop and implement policies and programs as necessary
  • Assist Outreach Team to develop community relationships with Hospitals, School Districts, Primary Care, etc for referrals and continuity of care

Schedule:

Clinic hours of operation are M-F from 8a to 8p, and Saturdays 9a to 5pm.

Job Types: Full-time, Part-time

Pay: $28.00 - $32.00 per hour
